I don't think it was valid for rebate. I've earlier rebate form & it shows clearly valid from 9/28. You might wanna return that item back to Macys.

Not possible. You might have ordered on 9/11. Note that their server time zone is EST, so if you're living in PST and order it on 9/10 9 PM onwards, you're essentially ordering it on 9/11

As long as the barcode has the 12 digit UPC with it, that's what you'll include.
You should've been emailed an invoice, or go to Macy's website to print one.
