The Sweepstakes begins on February 01, 2017 at 8:00 a.m. Eastern Standard Time and ends May 31, 2017 at 5:00 p.m.

Volcano Bay Family Vacation Package (the "Sweepstakes") is open to legal residents of Florida, who reside within in a fifteen (15) mile radius of Simon Orthodontics, (10115 Forest Hill Blvd, Suite 301, Wellington, FL 33414 or 3472 Forest Hill Blvd, Suite 1, West Palm Beach, FL 33406 the "Sweepstakes Area"). Void outside the Sweepstakes Area.

There are three (3) ways to enter. A.) Referral, B.) Begin Orthodontic Treatment, C.) In-Person,
Onsite Entry.
a. Referral: To receive a referral entry, you must refer a new patient to Simon Orthodontics, who schedules and completes an initial consultation during the
Sweepstakes Period. New Patient must identify you as the referral source when scheduling the consultation;
b. Begins Orthodontic Treatment: Each new patient who executes a contract and commences treatment with Simon Orthodontics within ten (10) days of their
completed, initial consultation, and during the Sweepstakes Period will receive one (1) entry, if initial consultation was completed prior to the Sweepstakes Period and
the new patient did not commit to recommended treatment, such patient will have thirty (30) days from the Sweepstakes Start Date to execute a contract and start
treatment; or
c. In-person, Onsite Entry: This method of entry will be available at Simon Orthodontics, 10115 Forest Hill Blvd, Suite 301, Wellington, FL 33414 during normal
business hours on any non-holiday weekday during the Sweepstakes Period.
8. Limit twelve (12) entries per person, per household for the duration of the Sweepstakes Period, regardless of method of entry. Entries received thereafter from any such person will be disqualified. (Their website says unlimited entries.)
All become the property of the Sponsor and will not be acknowledged or returned.

.Grand Prize (the "Sweepstakes") – Accommodations with hotel stay for up to four (4) occupants, for two (2) nights, and a four (4) pack of tickets to Universal's Volcano Bay in Orlando, FL 32819
("The Theme Park"). Trip details:
a. Winner is responsible for ground transportation to/from Universal's Volcano Bay, Orlando, FL 32819. Sweepstakes winner and additional individuals must provide all
documents required for entrance into the Theme Park.
b. One (1) standard room (quad occupancy, two full/queen beds) for two (2) consecutive nights at an Orlando area hotel. Based on availability at the time of the
booking (room and tax only). Final hotel selection shall be determined by the Sponsor.
c. Four (4) single day/single park admission tickets to Universal's Volcano Bay, in Orlando, FL 32819.
Restrictions
10. Entire trip must be completed by December 31, 2017.
