YOU DON'T NEED TO STILL OWN YOUR PS3 TO GET THIS. JUST SUBMIT WITH YOUR PSN NETWORK SIGN IN ID THAT YOU USED BACK THEN. FILLING OUT THE FORM CAN BE DONE IN LITERALLY UNDER A MINUTE.

Seriously though, other OS removal and the yellow light of death on these models....I wish it was a bigger payout for us.

I couldn't care less about the other OS stuff but I did have to replace the laser 3 times on my PS3 (twice at my own expense)
Was still playing games off the HDD when the disc drive failed thanks to GeoHot, then Showtime basically brought all your movies and games into one app.....Man, those were good times....
