I can't be 100% sure with Sprint, but generally it's the 1st line listed on a bill with multiple lines.
Thanks much.

1. Go through deal URL
2. Bring your own phone
3. Port a line

Max two GIft cards per account.

The deal is best if you are porting two lines. Monthly $80 per two lines of service. in three months you spend $240+ tax and get $500 GC

AT&T taxes & fees are high, around $20/month for 2 lines. Your monthly bill will be around $100.

Log into your account and look around, a Chat button will pop up, Click on it and tell it "agent", it'll connect you to a rep. Ask the rep to combine your lines into one plan. Afterward, ask the rep to confirm that your account has the $250 offer attached to it. Then email the transcript to yourself by clicking on the "envelope" icon at the bottom of the chat box. Do that before closing the Chat box.
Thanks and repped...Not sure why they add 2 plans when i selected one plan for both lines.

2nd order (wife's name/ssn) got cancelled.
The 2 lines are porting in from Sprint account in my name.
listed all the correct info in carrier-info section. ie., att account on wife's name, and carrier-info corresponds to my info.

is there anyway to get this order to be successful?

my wife had a att account in 2014, so i was hoping it won't fail with "could not verify the order came from you"

I don't know if they do it on purpose, if you don't get it corrected then your bill will be higher because the first line in a plan has a higher cost.

Try again I guess. It's definitely not cancelled because the address is blacklisted.

Right got the ship notification just like you mentioned yesterday for my 1st order on my name, with email "Incoming! We shipped your order xxxx"
so, the address is not blacklisted.

any recommendation on what i should use for carrier info (name, ssn)
is it worth to chat or call to see if the order can pass through, after i try again?

Your wife can call E-comm at 866-267-4510 to verify her ID and billing info, etc. It wouldn't hurt the chance, but doesn't guarantee success.

What did you see for 'credit check' status while placing the order?

Ok, then it makes sense to try again. Before that, try to reach out to them and understand the reason for cancellation. One thing they focused on was that the account address, shipping address and credit card address should be same. Also, if your Sprint number was no credit check, the SSN for port-in doesn't matter.
