Mine came without ear tips, twice. So that's actually not an issue. It seems they throw away the old ear tips and replace them, but not replace them well.
After my replacement was also missing medium size I contacted Apple under a warranty claim. The rep said that they really shouldn't replace them as missing pieces wouldn't be covered under the warranty but threw me a bone anyway which was nice.
