1. If you have a current Xbox GPU subscription buying this Xbox Live card for 3 months will convert to 50 days of GPU.
Alternatively ...
2. If you do NOT have a current GPU subscription, buying this Xbox Live card will give you 3 months of Live. You can then add on a month of GPU for $15. Once added, you will have your 3 months of Live converted to GPU. Of course, to maximize the savings, you would really want to max out your Live at 36 months before throwing on the GPU.
This is confusing to many though. If you aren't following these XBox Live conversion to GPU threads you may not understand the differences between having a sub vs not having a sub and how the actual conversion works.
Finally, from a math perspective ...
If you have a current sub and you want to extend your sub out 3 years the total cost for that with this deal works out to approximately $220 ($10x22 50 day conversions). If you have an expired sub or are a new subscriber, those same 3 years will cost you $135 ($10 x 12 3 month Live cards + 1 month GPU @ $15 for the conversion).

Looks like they take another 10% off but the item still says Gold and not ultimate. Am I missing something here?

It looks like you can get up to 10 subs on this deal (per purchase?). So 12 or 22 would require multiple purchases?

It is worth noting that microsoft could terminate gold conversion at any time. While you would get more out of them by waiting for your subscription to lapse, they may become worthless for gamepass between now and then. I'm not claiming it will, but just keep it in mind when deciding to save codes for gold.
