Even after reading this form, I have only a guess what it is all about.
My guess is it is a request to find out if and/or confirm you are a beneficiary for a deceased, alien spouse's (not citizen of US) benefits.
Please explain your scenario in detail.
When and why would someone need this form, and what will it accomplish?
I know you posted as a PSA but it is not helping anyone if we don't know what it is about and why we might need to know.
