Last night I made use of the promo code and ordered a Pepperoni and Old World Pepperoni two topping custom round for $5.99, which is nearly half of the regular pricing. So, I'm not sure if you're missing something, but this deal is a pretty significant savings in some scenarios. If you're comparing a two topping custom round fresh out of the oven it to a Hot and Ready Classic Pepperoni, perhaps it's not the hottest deal, depending on the pricing of your local store.
In my opinion there's a fair difference between the products and I consider this to be a pretty decent deal, especially when considering the lackluster deals/promo codes Little Caesar's has been offering the past several months.

It's a hotly contested issue on the Little Caesars sub-reddit as to whether paying $1 more for an ExtraMostBestest is a rip off or worth the extra cost. It's worth it for the additional cheese, the way my store prepares them. Little Caesars has very specific guidelines about how much more cheese and toppings to use, but there will obviously be variance between locations. Imo, now more than ever the $1 seems to be worth the additional cost, seeing as the overall cost has increased and it's a smaller percentage of the total cost.
