That's like the recipe reviews that say "I substituted half the sugar for apple sauce since I thought it was too much and used bread flour instead of all purpose since I was out and these muffins just didn't come out right. 2/5 stars"
It does. It monitors the charge state (V, mV) of your car/truck battery, and when it detects a drop in voltage, should send a corresponding current, up to 1 Amp, to top up the battery. The main use case is if you don't run your auto for more than a few weeks (eg. travel, use other transit), it would help to ensure your battery remains charged, especially when another low parasitic charge is being drawn, like a car alarm or other electronics that would end up discharging your battery over time.
It's basically a trickle charger. Sends 1amp to the battery to maintain the car battery at 12.5v. Will also keep power to the interior electronics when you change out a car battery to keep from dropping memory/presets.

Ideal if you have a parasitic draw on a vehicle that you may not drive very often.

It's ok if you don't understand 12v systems and what exactly a trickle charger is.

Good price but the terrible reviews should make you think twice. Though part of that may be user error, it seems like a lot of folks are using this as a long term solution which it arguably isn't designed to be, and hot swapping the batteries, which is apparently a no-no.

Lithium ion is a lot more energy dense that lead acid. A 50 amp hour battery has about 600 watt hours (50 * 12). A DeWalt 4 amp hour battery has about 72 watt hours (~18*4, 20 volts is the max not nominal). So that means an over 10% increase in energy. It's enough that if you leave your car in the airport for 3 weeks it is more likely to start up again.

I'm invested in BatteryMINDers and extension cords, and not in Dewalt batteries. $12.56 is not a high price to have this capability ... but not having a two-pin plug is a huge limitation, along with remembering to check the battery over time. Probably best just to consider this purely a "MEMORY SAVER- Maintains car settings while charging your car battery for systems such as radio, seats, clock and dashboard settings".

Yeah, the "airport" suggestion sounds good.

I think you nailed it.

I was trying to think of the use case, why would I want this instead of a trickle charger. And the answer would be when I didn't have any place to plug in a trickle charger.

Agree. What if the vehicle doesn't have OBD? Can we hack to just get 12v? I suspect it's "too smart" though

FWIW one of the actual main intended uses for this is for mechanics, you use these when swapping a battery so you don't lose presets and have to reinitialize windows and all kinds of other crap on a modern car if you lose 12v power for too long. At 13 bucks I jumped in on one for giggles and if I have to leave a car sit.

Yeah. Like a the airport if you going away for a while. If I have a place with sun, I just attached a solar one onto the battery.

Since is only 2 wires, you can just get a female ODB 2 port and connect that directly to the battery. You could also just cut the wire and attached the ends to some kind of DC plug so you can still use it with an ODB 2 later.
