Costco front-loads some of that in the membership fee, but item prices are generally cheaper and quality generally better than their competition. There's a lot of additional services you get access to as well through the membership which helps further justify it.
You pay Walmart (and other low-cost grocers) through your taxes (since their employees are almost always on governmental assistance) as well as low quality for comparable items. Grocers with similar quality have higher item costs per item.
Costco: Generic Zyrtec - 365 pills @ ~$12
Meijer: Generic Zyrtec - 300 pills @ $34
Meijer: Zyrtec - 60 pills @ $34 (or 360 @ $204)
Besides that, there are many other items Costco carries (nuts being another good example) which are generally better sourced and cheaper than anything you can find elsewhere.

Being married and in Texas, BJ's aren't available to me.
Wife has lost interest in doing it together so I end up going solo at Costco.
Sometimes I wonder if the Sam's around me could offer the same BJ's experience, but I have just never found myself very attracted to their appearance.
That said, we are Executive Members for the past 12 years. The return policy saves a lot of money. Just don't abuse it. Eighty percent of our purchases are for non perishable items such as Dyson, ranges, refrigerators, Refresh eye drops. You generally get larger quantities for the same price,
Recurring purchases.
My wife's refresh.
Healthy Choice fudge bars. At full price, about 60 cents each.
Tate's chocolate chip cookies. Large boxes.
Anti diarrhea pills, $ 1.25 for the large boxes.
No one beats Costco on these items, not even Walmart's Equate brand.

You had me up to your low paying government assistance quip. Let's see some facts to back that up or else you're just making it up.
The negative about Costco is the sparse shopping variety and the sizes you have to buy. For a small family or a couple you have to get creative with what you buy to make it worthwhile. Gas is one way but only if you have a Costco with gas near you or on your way. Depending on your milage you could spend more driving there than what you save. Some people are so set in their Costco ways they lose common sense and find a way to justify any trip there.

And I agree, Costco isn't a good fit for everyone. But their non-grocery goods are VERY good too and great prices. Some of the best deals on electronics with an extremely generous return policy. A chest freezer makes shopping there easy, most of their food is either frozen/freezable, has a decent shelf-life, or isn't that 'bulk'. Membership fee is part of what generates those prices, the slightly restricted selection is the other. Their procurement department is very good and very clever.
You're not going to get 100% of things you want there, but that's not the point. Most families make a big costco trip once a month or so and stock up on paper products, refill the freezer, and pick up what groceries you can. But it's hardly super limited either, mostly only having 1 or 2 brand options. But the brands are usually really solid and priced like the cheap brands at a place like Walmart.
My point is that it all sums up in the revenue. You pay for the items one way or another. The membership fee (a measly $65 per year...) effectively comes off from the discount price. I've never met anyone who hasn't saved many times over that at Costco.
