Probably because they weren't installed perfectly perpendicular to the edge of the door. I was lazy and didn't pre-drill pilot holes and when I put in the screws and it twist the hinge a little bit...

Same place people that build cabinets buy their hardware. For hinges and drawer slides, try CShardware.com. Not the easiest site to find things, but most cabinet makers know what they need already....
