It refers to frame/yoke /straps. I'm 5 9 190 and have 1 sm/Med and 2 large Osprey. I ended up replacing the straps on the medium with large straps from Osprey because it was too tight with layering....

Small/medium is the frame it’s intended for. I think under 5’8” ish would be good for the small/medium, whereas taller people would probably be more comfortable with a medium/large.

This backpack is huge. You could probably easily fit 2 dozen American Kestrels in there. I doubt you could fit more than one Osprey though.
