Apple does refurb iPhones. If you take your phone to the Genius Bar under warranty, and they can't fix it, they swap it out.. with a refurbished iPhone.
From time to time, Apple also sells them...

iPhones are refurbished by 3rd parties only. If there is a warranty it's the 3rd party or its just the balance of the Apple warranty from when the phone was originally activated. Am I the only one...
