5 orders placed and one promotional code received. Started a chat with Amazon about missing promotional codes and was told to contact Microsoft. Contacted Microsoft support and they told me they are offering a promotion through xbox.com but not Amazon and that Amazon will need to resolve. Contacted Amazon again and escalated the issue to customer service over the phone. Rep confirmed there is no limitation in the terms and conditions but said she is unable to assist with the additional codes. Escalated the issue to another internal department only reachable by email. Told I will receive an email from this department by end of business today with my codes or I could request a refund if they do not provide resolution.
Anyhow... here is the promotional restrictions for this listing for all those buying multiples like myself. Enjoy the great deal while you can. I did
UPDATE: Amazon provided 2 additional promotional codes and refunded my purchase on the last 2. 26 months of live for $45 is not too shabby.
Cleaned up the attachments by deleting the small screencaps and adding high resolution pics of the original listing, promotion wording with restrictions popup and the promotion as appears in cart to go along with the restrictions popup I posted earlier. Anyone indicating deal was advertised as one per account is not being truthful.

After i placed the order, and the order was verified. The original 3 month code shows up in my games and software library.
the bonus code shows up in email, from account-update @ amazon .com

Edit: only hair works once.
Tip: when I added my first code it gave me the option to enable Auto-renew for 1 free month. Then I went and turned it off. It worked once or twice more, can't remember exactly, but it did stop offering eventually.
3+1+3+1= 8 months total for $16.04 after tax. 5% CB on amzn card so $16.04 - 0.75 = $15.29 / 8 months = $1.91 per month. Slick!
