What you should have done (and could do in 30 days if you wish to reapply) is to simply apply as a "Sole Proprietor". From that point, pick a category/subcategory that makes sense for you, put minimal levels of income and years in business. I have gotten 3 Chase business cards doing exactly that (2 using my SSN, the 3rd with an EIN), as have a large number of other people - oddly enough, it is easier to get approved following those steps than for legitimate small business owners applying for their businesses.
One thing to know, the buyer protections granted to customers of non-business VISA cards may not come with the business card. You may want to check carefully before using this card for higher risk purchases.

Funny, I was sent an email stating I was approved so I had them expedite the card. I check the mail today and there was the chase ink card. UPS drops by and gives me another chase ink card that was expedited. Now I have 2 cards.

TLDR: Applied with my name, sole propietor, they asked me to mail proof of my SSN, and I'm approved now.

Sent a message and told them to expedite the card to me.

Business name = your own name.
And if you already have a chase account, the new account will be active and ready to use within 24 hours without even having the card. Just login to your chase account, send a secure message and select your new card, then after you sent a message to chase (doesnt matter what you say) Look at your sent message and your full CC account number will be on the card. The card expiration is always 5 years from the current date so 4/23 will be the expiration. You can't get the CVV code though, but lots of places dont need that (amazon for instance)
