What do I need to do?
Purchase a new qualifying Apple iPhone X, XR, XS, or XS Max at Full Retail Price (plus tax) during the promo period.
Activate at least 2 new voice lines for new customers or add on at least 1 new voice line on an existing account. Any voice rate plan is eligible.
Trade in a paid-off, eligible Apple iPhone 6, 6 Plus, 6s, 6s Plus, 7, 7 Plus, 8, 8 Plus, or X in good condition.

General information compilation

Here is a sheet guide based on information I collected today on this deal, both in-store, online, and talking to reps to the best of my knowledge

Steps:
You must have an old iPhone to trade in to use this deal. If you don't have one, you can't get the deal.
It is recommended to use the cheapest and oldest iPhone (an iPhone 6) to take advantage of this deal, as you won't get much back out of it. Most estimates are around $50-60 for the phone in good, working acceptable order at this time.
You need to be ready to open (2) new lines on a T-Mobile account, or add (1) new line on the T-Mobile account. You must be the original account owner, or be an authorized user on the plan to make changes on your account.
When you are ready with both 1 & 2 from above, you are ready to go buy at Costco.
At the wireless kiosk and you have the option of purchasing the following, which seem to be mostly 64GB available at the moment. It is unknown if all stores are carrying 128, 256, 512GB versions.
iPhone X (if in stock, not all stores have it available)
iPhone XR
iPhone XS
iPhone XS Max

After you have made your choice, you can either get the option to make the new t-mobile account or add one to your existing account. It runs about $120 for 2 lines at the general plan, or ~$40 (I think?) for adding a new line to an existing account.

Per Jonnyjett: OR, if you plan on cancelling the service as soon as possible, and want a plan that costs the least amount of money, then sign up for the "Talk and Text Only Plan", which is $20/line and includes all taxes and fees. Kiosk may have to call Tmo to get this option.

If you want to add it to an existing account, you have the option to drop an existing line/phone number and you can re-add this promotion as a new number so you don't have to pay for an additional line. <<<< not sure definitions of "drop" but please note the

Per Jonnyjett: If you have a good grandfathered account, with perks that you'll lose by adding a line, or you have a "business account" or "business rate" (which is code for discounts that can't be replicated and a plan that can't be messed with), then request either a whole new account, or request a "sub account". With a "sub account" you can have a plan like the "Talk and Text Only" Plan, and it won't screw up your grandfathered plan. And you can cancel your sub account without any ramifications to your grandfathered account. Kiosk may have to call Tmo to get this option. The only question is, does a sub account require 1 or 2 new lines? I opened 2 to be safe. Besides, you make a $50 Costco Cash card by BYOD on the 2nd line. So you're $10 ahead on the deal.


You cannot add a new line if you are at the limit on your plan (10 for general family plan)

Kiosk rep will print out invoice for you to pay at cashier.
After you pay cashier and get new phone, go back to Kiosh rep.

The Kiosk rep will verify your trade in, and input the IMEI and information of the trade in phone.

At the end, Kiosk rep will print out a Shipping label and receipt with IMEI of old phone.

Give it 24hrs, then register for rebate online: https://promotions.t-mobile.com/OfferSearch (Enter new line info)
You ship off the old phone + receipt inside box on your own.
Just ship the phone, nothing else. Make sure you wipe it (Find My iPhone is turned off).

The trade in is worth -$x off your rebate. i.e., if you get $50 for your iPhone 6, you will get ($750-$50) a $700 rebate gift card in return.
The rebate comes in a credit giftcard, mastercard from what I have noticed. Some other options people are getting is $750 in bill credits instead.

exact official wordings of 90 days clause:
Since a new line activation is required for this offer, you can't cancel a line with the intention of activating a new line just to take advantage of this offer. If a line has been canceled within the past 90 days, it must be resumed before you can add a brand-new line to qualify.

Last time I did this deal in November, the terms and conditions said I had to keep the line open for 180 days but they sent the rebate card in a month so people were canceling lines before that

No - it did not say you MUST keep it open - the wording was something along a suggestion or something like that. Many reps said you did need 180 - but has you have written, that was also bogus.

Yes, they have now added the Iphone 6 which makes this deal even better as you can find these real cheap on message boards, neighbors or in your drawer - LOL.

As to end date, see the first post. The link to T-M says this: "March 20, 2019 through TBD"

Limit 1 per account.... so essentially this a BOGO if you need two lines. Costco T-Mobile is already offering $390 per new line with trade-in. Maybe I am missing something, but this deal is nothing new if you need 2 lines.
