Comes as one code for 24 months. Also you can only stack 3 years of xbox live. I know someone who tried to redeem 2 of these but couldn't due to the 36 month limit.

I say all of this from experience. I was pretty annoyed when I tried to apply 3 years of cards, but because I had 2 remaining months on my account, I couldn't apply all of the cards.
