I placed an order, it was cancelled. Placed again, was cancelled again. Called costco and spoke to Order Support Team.
They basically said since it's a virtual delivery code, some people get flagged as fraud and get cancelled. What you need to do is call them, speak to Order Support Team and explain the situation and they need to verify your information. They will verify your membership start date, your DOB, and your address and how long you lived there and your previous address that is not on Costco's order page.
After that, ask them to place a manual order with the same card number and transfer the pre-authorization to that new order so that they don't place another pre-authorization amount on your account. I did that, and got the Xbox code within 5 minutes.
*You only need to do verification once in your lifetime, future virtual code orders won't get cancelled.
