Terms: Offer expires 9/7/19. Requests must be postmarked by 9/9/19 and received by 9/18/19. Reproductions of certificates, UPCs and cash register receipts are not valid. Please allow 8-10 weeks for delivery. Refund check must be cashed within 90 days of issuance. Limit two (2) rebate requests per family or address.
