Order must be made at Costco kiosk. The kiosk will ship it to your address.
Device must be financed with installment plan ($25*24 months). Only tax will be due at the kiosk.
To request sim unlock, device must be fully paid off (installment plan can be paid off early) and be active on sprint network for 50 days.
Rebate form must be submitted online within 30 days. Rebate will be fulfilled within 14 weeks.
Other phones also qualify for the offer. But I chose to post only about XS because it's the best value.

The deal is in-warehouse only, not online. Must be existing (upgrade) or new Sprint subscriber. Every Sprint subscriber with a compatible, regular plan that can be dealt with by a national retailer (no SERO, SWAC, Free Year, Kickstart, etc.) and eligible upgrade can get this. Costco Wireless Advocates kiosk will ship the phone to you and print out a rebate form. You must file for the $500 Costco Cash Card online using info from that form.
Phone MUST be financed on 24-month installment. No outright purchase or 18-month lease.
If your account is a no credit check account, you cannot finance phones.
If your plan has an ongoing promotional discount for BYOD and NOT financing a phone, it will disappear when you finance one and revert to regular rates.
If your account / line which was upgraded is canceled before $500 Costco Cash Card is issued (in 14 weeks), rebate may get denied.

i made the thread to let people know XS has $500 rebate now. it didn't qualify for any rebate before today.
so the detail about XS is buried inside 100 pages.

also, it's (back?) in stock now. they had ~150+ XS in each color when i went this evening.
For the latter, Google Voice is your friend, or just suck up losing out on current plan and transfer your real number from the start. I am suffering now with Sprint 5 lines for $100 Plus, which isn't available anymore, and none of those 5 #'s are my main lines that I need on the plan
You can always get a new number on existing Sprint line, though they give it to you (you can't port in a replacement # etc).
You can add your real # as an additional line, and after gift card arrives, cancel original line. Depending on circumstances and # of lines and/or promo line costs, this might not bother you. i.e. if you only had one line, added your real line and, then dropped initial line no biggie. But if signing up for 5 lines (free 3,4,5th lines for a year) then cancelling a line will have big impact on monthly costs.
