Try using your Amex card instead. Might have better luck.
Never seen that. Companies that do not want to accept Amex don't sign up for it in the first place as it costs money to sign up for processing and the like. It also makes no sense to sign up and then not accept customer's cards and piss them off.
As to the fees, like with any business expense, it is passed on to the customer. Merchants that don't want the added expense simply do not sign up to accept the card and go with Visa, MC or Discover or do not accept credit cards at all. There is a reason why many small businesses will give you a discount for cash. Namely it costs them less (no CC processing fees) and let's be honest, they can hide the income off the books more easily.

Maybe, but if they do not want to pay the extra amount for an Amex because it came with some sort of package processing deal, they will simply not show the Amex logo in their store and claim they don't accept it, not show the logo and then try to talk you into using a Visa (unless the owner is an idiot imo). Yes there might be the occasional person that goes onto the Amex website and looks it up online for Amex in their lame small business database, and all the merchant has to claim is they are mistaken\no longer accept it.
What also pissed me off are most Asian stores in Little Saigon area in Southern California will refuse any cc if the transaction is less than a certain amounts: less than $8 or less than $10 and I never have cash on me.
