$193 is what you get when they receive your trade-in and they confirm it's in acceptable condition.

The balance of the $415 will be spread across the 30 months as credits. That's how T-Mobile does their trade-in when a special offer is involved.

Not for the same line, no.
You'll have to buy an iPhone from Apple and another iPhone from T-Mobile to do that.
