This is my only exposure to capital one and I must say it seems a little shady. When you have an advert that publishes get $400 for opening this account and doing xyz... and you click the link and sign up... you shouldn't have to worry about some promo code in an obscure box somewhere making or breaking your deal. My guess is they were hoping many would sign up and screw this up so they get a flood of new banking customers and not have to pay many of these out.

Yep under $10K you dont have to pay capital gains
Most banks send you a 1099-INT Tax Form (INT = INTEREST) which will list the interest + bank bonuses and you pay tax on that at your ordinary income rate (whatever brackets you fall into)
Capital gains rates.. both short term and long term refer (usually) to the sale of investments. However it is noted that someone's short term capital gain rate may also equal their ordinary income tax rate.
