I was just at Costco and they told me today is the last day for this deal. I am going to wait to see if a better deal is released as they told me that when one deal ends, shortly after another begins. They had plenty of 12's in stock.

Got 2 Iphone 12 Pro from my local costco today. They got 5 Iphone 12 pro today. Traded in a 1st Gen Iphone SE and an Iphone 6s.

This was for an upgrade. I kept same T-Mobile One plan I have. $140 for 4 lines (including taxes and fees).

Iphone 12 Pro (128 gb) is $899 ($100 less than t-mobile)
Iphone 12 (64 gb) $746 (additional $50 for a 128gb) - this is $80 less than when you get it from t-mobile.

You get more trade-in value if you add a line or port your number. For upgrades, I got $194 each for an iphone se (1st gen) and iphone 6s.

Paper works I got says "Promotion Dates: 10/23/20 - 12/10/20". See attached.

Do you know if it will stack with byod 200. In your case you did not add a line.so no byod

Not sure, but I know that you get more trade in value if you port your number from another provider and there's an extra $100 or $200 costco cash card (if you add a line or port your number, not really sure).

Do i need to use the new phone and be on active line to get the trade in rebate back? I am thinking about keeping it in box without using.

I'm pretty sure you don't have to activate or use it. You just need to send in trade-in phone within 30 days to get the cash back.
