Real sam's club GC for $45, which can be used at both walmart and sam's club. So basically free membership
Yes we got a physical $45 card.
Thanks for confirming, can we use that gift card for the same day purchase once i receive it, I don't like to go multiples visits as my sams club too far from my house

Why not? Just go to the member desk first to collect the GC. And then you are free to shop around using the same GC as needed. I mean it's just a regular store GC, no strings attached
