I looked the book up on Amazon on my computer and looked at the URL to see the item ID (looks like "B00514HX3F"). Then I said "Alexa, order B00514HX3F". I doubted whether it would work, but it did! Alexa read off the correct title of the book, indicated that the $5 promo had been applied, and confirmed that I wanted to order it.
Hope that helps anyone who might want to use this promo, but is struggling to get Alexa to cooperate.
On any device (including the Alexa app), say this phrase:
She should respond with "(Your book's title) by (your book's author). The Kindle ebook is (price) after a $5.00 Alexa-exclusive ebook credit. Would you like to buy it?"
I've tested a few different books on Echo Dot 3, Echo Show, Echo Show 5, and my Alexa app. This should be foolproof!

I was able to stack my no-rush shipping credits towards the price. So I bought a $10 priced Kindle book, got the $5 off, and used $5 of my no-rush credits. Net price, $0.00.
Not for me, but it's something to be aware of. I had $6 in freebie credits, and thought similar to you, so used $4.99 on a movie I wanted just to be safe -- had the same fear they'd get used up first.
Had $1.01 leftover, then did the $5 kindle book thing, and I still have the same $1.01 leftover. So it didn't grab my slow shipping credit at all.

My point was that if it's a $5 purchase with $2 of slow ship, it'll do $2 slow ship first then $3 promo. So you're left with $2 promo and used up your slow ship. You still don't make sense, that's not your use case.
What was being asked is what happens if the order is under $5, which credits get grabbed first. And in my case, it was the kindle promo here as it didn't touch my no rush credits. But in my case it was a 3.99 book, not $10 -- if the book is over $5, and if the buyer has no rush credits, I expect they'd get eaten up. And that's what I assume most would want.
I'm not sure what criteria amazon is using.
The book I purchased is sold by Penguin Random House. And the promo worked fine for me.
I would guess if a title shows as applying no-rush credits, the book is eligible for promotions -- and if it doesn't, maybe not?
Regardless, Alexa told me the promo would be applied before I purchased, with zero due (as it was under $5)... so if folks don't hear that bit about $5 being taken off, don't order.
