Port to Google Voice (one time $20) & have the number forwarded to up to six phone numbers & unlimited devices with the app installed. Lets you take advantage of any provider deal regardless of existing customer status by just adding/changing forwarding to the new number the provider gives. Bunch of other features as well.

I do bank bonuses to generate side $$ so I have had about 25 checking/savings accounts in the past year. The only account I have run across that struggles with google voice and 2fa is ally, which is a pain in the ass because it is my main hub, but they allow a code to be sent to your email. I am guessing google voice must have made good progress because it is a non issue for most banks I deal with now. I can't comment on prior to August of 2020 since that is when I ported my number to google voice.

Us bank and Wells Fargo are no go as well. Source, just tried.

That is correct, but you run the risk of losing the plan if you wait too long. I have activated 40 days after ordering a SIM -- not the StackSocial plan, but a half price plan directly from Tello. I was afraid to push it longer after receiving an email from Tello that the number they assigned was going to expire.
I purchased the tello 6 month plan via stacksocial on an earlier promotion on May 11, 2021. So now on June 16 it is about 36 days since purchase. I have not created a Tello Account or claimed my code on StackSocial yet. Since it is more than 30 days after I purchased the plan, I'm concerned Tello/StackSocial may deny the 6 mo svc plan. I'm okay either with getting a refund or signing up for a 6 month plan, but concerned I may simply be denied a plan and lose my purchase price. What is the best way to proceed here?

Sorry, I don't really know. Someone recently posted that you can get a refund from StackSocial if you haven't claimed the code. That would be the first thing I would try. If that doesn't work, I would claim the code and try to redeem it at Tello and hope the 30 days isn't a hard deadline. Let us know how it goes in case someone else here has the same problem.

From experience, 30 days is not a hard deadline but I wouldn't stretch it.

Click "claim my code" and Stacksocial site will display all the instructions to follow when redeeming code on Tello site. A credit card or Paypal is required for placing the order (it won't be charged). When I tried to finalize the order, Paypal wasn't being accepted. Instead, I used Paypal Key (Paypal's virtual cc).

Once order has been placed the SIM card will be shipped.

Worth noting, you have to wait for the sim card to arrive to activate. I had another sim card which i tried to activate and it didn't work.
