Ported from Cricket instantly and submitted both the Costco and T-Mobile rebates. Called in for T-Mobile rebate and didn't mention anything about Costco and he confirmed that my account is fully eligible and qualifies for the $150 rebate. $450 total to switch and stay on for a few months is not bad at all.

I would like to port a number to one of lines I set up for this deal. Does anyone know if that has caused issues in the past? My local WA kiosk guy says it should be fine, but...

Did anyone port in from t mobile MVNO and submit port in offer for $150 per line? I tried chatting wth t mobile support and that person had no ides about any port in promotion .

yesterday I went back and manager was able to generate the rebate form by submitting things in PRISM. when i came to my home, the IMEI on the form was not right, and while submitting rebate on mypromo I was getting IMEI error when using what they had put. so I went ahead and used IMEI showing i tmobile site . Anyone faced similar issue ? any suggestions on getting it fixed or should it be fine

I was able to receive my rebate even with wrong imei .... i opened new lines and provided wrong IMEIs ... turned out those IMEIs were Verizon phones and not working with Tmo ... I then used different phones with different IMEIs but I still received my rebates

It does not matter what is the IMEI # of the actual phone you are using. You need to match the IMEI# on the paper works. (The print out and the one you input for rebate)
