It can be any netflix account, if i remember correctly you get a link on your email for the credit and all you would do is click that link and sign in the with the netflix account for which you want the credit. I have purchased two and use it on a different Netflix account.
Chromecast has been the best i have used so far.

I guess this won't help if you have Netflix through T-Mobile, or will it remove the upgrade fee for the better Netflix plan?

It just added the credit to the Netflix account so you should be able to just upgrade for difference like Disney+ and Hulu with Verizon.
