That is why you were denied, the 0 utilization. Cap is notorious for wanting to see utilization now. They will even ask in recon why you are applying since you apparently have sufficient credit that you aren't already using.

One issue I have with Cap1 though is the 2:1.5 transfer to Eva Air which I really wish could become 1:1 like Citi. I can certainly transfer to Avianca to redeem for Eva tickets but the availability is much more limited. In fact even United seems to have better availability in terms of Eva tickets so Chase UR points are easier to redeem IMO. (Hyatt is also a good option.)
Yes they do. I finally got mine in the mail after two weeks or so. It takes about 5 days to Priority Pass to recognize your CC number to register in their system and apply for the card. Other than that it's pretty easy if you're not in the rush.
