United Healthcare through Level 2 offers free Dexcom G6 CGM system/meters and a free Fitbit Inspire 2 to those with type II diabetes. Insulin is NOT required. All you have to do is call the number and/or fill out the form and inform them you have been diagnosed with type II diabetes and would like to enroll. Having United Healthcare is required. You do need to use their Level2 Dexcom Zone tile app on your compatible phone.

Why is this a deal? Because many insurance companies require prescribed insulin before covering CGM meters. Most are left to pay out of pocket. A month of Dexcom sensors runs $449 and you have to buy a $299 transmitter every 3 months besides the sensors. You can go with a FreeStyle Libre 2 CGM meter, they can be found for around $70 a month if you use GoodRX. Many like like that system better. Dexcom prices can be found here:

What is a CGM? They are small device that sticks on your skin for 10-14 days, with a small probe that sits under skin and continuously measures your glucose levels.
Bonus: United Healthcare is giving extra incentives depending on your plan. One is a $750 yearly deductible credit for wearing the meter for 20 days a year. The other is $0 covered co-pays, drugs, etc. You have to ask them which incentives you qualify for. From what I understand they do not stack.
Update 1: From what I understand the "Choice Plus" plans qualify for $750 deductible and the $0 Co-Pays are for Bind plan members. See attached.

As someone who went through multiple daily injections, I highly recommend checking out afrezza, if your insurance will cover it.
Kaiser will cover Freestyle libre (at least in SoCal), but there are multiple requirements you need to meet, one of them is being on insulin and has frequent hypoglycemic episodes. It is an HMO health plan so they keep cost down as much as they can I.e. no go on expensive dexcom if cheaper Freestyle serves the purpose.

From looking this up it seems like you need to be taking insulin to be eligible for medicare coverage.
Tes they do. I have kaiser and have been on Dexcom for 6 months. Copay is like 60$ every 3 months. I have type II, it is a lifesaver.

The Dexcom G6 Glucose Monitoring System (K0553, K0554) will be considered for reimbursement for adult members when the below criteria are met:
• The member has established diagnosis of Diabetes Type I or Type II
• Medical necessity criteria are met (American Diabetic Association -2018; Centers for Medicare & Medicaid Services 2018 or NCD/LCD; Interqual
• The member is using a blood glucose monitor (BGM) and performing frequent (four or more times a day) testing; and,
• The member is treated with insulin multiple (three or more) daily injections or continuous subcutaneous insulin infusion (CSII) pump; and,
• The member's insulin treatment regimen requires frequent adjustments based on SBGM (self blood glucose monitoring) or CGM testing results; and,
• The member has frequent (at least 2 episodes within 30-day period) unexplained episodes of severe hypoglycemia (blood glucose less than 50 mg/dl) despite member's compliance with diet and insulin regimen adjustments/ OR hypoglycemia unawareness, requiring another person's resuscitative interventions (e.g., glucagon administration).
• Within six (6) months prior to ordering the CGM, the treating practitioner has an in person visit with the patient to evaluate their diabetes control and determined that criteria above are met.
