United Healthcare through Level 2 offers free Dexcom G6 CGM system/meters and a free Fitbit Inspire 2 to those with type II diabetes. Insulin is NOT required. All you have to do is call the number and/or fill out the form and inform them you have been diagnosed with type II diabetes and would like to enroll. Having United Healthcare is required. You do need to use their Level2 Dexcom Zone tile app on your compatible phone.

Level 2 Program:
https://mylevel2.com/

Compatible Phones:
https://www.dexcom.com/compatibil...e-cgm-tile

Why is this a deal? Because many insurance companies require prescribed insulin before covering CGM meters. Most are left to pay out of pocket. A month of Dexcom sensors runs $449 and you have to buy a $299 transmitter every 3 months besides the sensors. You can go with a FreeStyle Libre 2 CGM meter, they can be found for around $70 a month if you use GoodRX. Many like like that system better. Dexcom prices can be found here:
https://www.diabeticwarehouse.org...e?q=dexcom

What is a CGM? They are small device that sticks on your skin for 10-14 days, with a small probe that sits under skin and continuously measures your glucose levels.

Bonus: United Healthcare is giving extra incentives depending on your plan. One is a $750 yearly deductible credit for wearing the meter for 20 days a year. The other is $0 covered co-pays, drugs, etc. You have to ask them which incentives you qualify for. From what I understand they do not stack.

Update 1: From what I understand the "Choice Plus" plans qualify for $750 deductible and the $0 Co-Pays are for Bind plan members. See attached.

Update 2:

Do Kaiser gives any CGM? I would appreciate if someone can give share their experience.
I have carried Kaiser Medicare Advantage for about seven years, started using Dexcom G5 in 2018, and have been receiving the G6 for the last couple years. Monthly cost is about $50 for both the sensors and transmitter. But, as many have suggested, each coverage plan is different. Edit: A CGM is worthless if you don't treat it as though your life depended on it - which mine does.

These can be hard for T1D to get covered on some plans (and for those people these are essential), so to get it for type 2 is impressive.

This is YMMV. I tried to get into this but my insurance plan would not cover it even though I have United Healthcare.

That's awesome! I'm on Cigna as a type 1 with multiple daily injections, spent months on the phone with Cigna, evicore, and edgepark and wasn't able to get a dexcom covered. CGMs are awesome take advantage of this if you can!

As someone who went through multiple daily injections, I highly recommend checking out afrezza, if your insurance will cover it.

Kaiser will cover Freestyle libre (at least in SoCal), but there are multiple requirements you need to meet, one of them is being on insulin and has frequent hypoglycemic episodes. It is an HMO health plan so they keep cost down as much as they can I.e. no go on expensive dexcom if cheaper Freestyle serves the purpose.

Everyone on Medicare can get a CGM, the only thing that will change would be the cost for them(if you have Medicare and full Medicaid, you shouldn't have any cost for a CGM and the sensor/transmitters). CGMs are covered by the durable medical equipment DME) aspect of Medicare and Part C MA plans, and all you need is a doctor's note and DME provider to get one. There are a few insurance carriers that are very liberal about providing CGMs at no cost, but this can change from state to state. UHC is not one that comes to mind based on experience with Medicare plans and my own personal health plan I have with them, but I will have to give this a go.

The enrollment form isn't working for me either:
"That didn't work, try again."

How is it searching for people?

OP - maybe make a step-by-step version of your post, you have a lot of info in there.

I'd like more details on this if you will. My mom is medicare with aetna supplemental and with type 2 diabetes and multiple injectable insulins and they want to charge her something like $300 for a CGM and sensors n Georgia. I thought it would be covered better than that.

I'll PM you. thanks!

From looking this up it seems like you need to be taking insulin to be eligible for medicare coverage.

Tes they do. I have kaiser and have been on Dexcom for 6 months. Copay is like 60$ every 3 months. I have type II, it is a lifesaver.

FYI the typical requirement are:

The Dexcom G6 Glucose Monitoring System (K0553, K0554) will be considered for reimbursement for adult members when the below criteria are met:
• The member has established diagnosis of Diabetes Type I or Type II
• Medical necessity criteria are met (American Diabetic Association -2018; Centers for Medicare & Medicaid Services 2018 or NCD/LCD; Interqual
• The member is using a blood glucose monitor (BGM) and performing frequent (four or more times a day) testing; and,
• The member is treated with insulin multiple (three or more) daily injections or continuous subcutaneous insulin infusion (CSII) pump; and,
• The member's insulin treatment regimen requires frequent adjustments based on SBGM (self blood glucose monitoring) or CGM testing results; and,
• The member has frequent (at least 2 episodes within 30-day period) unexplained episodes of severe hypoglycemia (blood glucose less than 50 mg/dl) despite member's compliance with diet and insulin regimen adjustments/ OR hypoglycemia unawareness, requiring another person's resuscitative interventions (e.g., glucagon administration).
• Within six (6) months prior to ordering the CGM, the treating practitioner has an in person visit with the patient to evaluate their diabetes control and determined that criteria above are met.
