United Healthcare through Level 2 offers free Dexcom G6 CGM system/meters and a free Fitbit Inspire 2 to those with type II diabetes. Insulin is NOT required. All you have to do is call the number and/or fill out the form and inform them you have been diagnosed with type II diabetes and would like to enroll. Having United Healthcare is required. You do need to use their Level2 Dexcom Zone tile app on your compatible phone.

Level 2 Program:
https://mylevel2.com/

Compatible Phones:
https://www.dexcom.com/compatibil...e-cgm-tile

Why is this a deal? Because many insurance companies require prescribed insulin before covering CGM meters. Most are left to pay out of pocket. A month of Dexcom sensors runs $449 and you have to buy a $299 transmitter every 3 months besides the sensors. You can go with a FreeStyle Libre 2 CGM meter, they can be found for around $70 a month if you use GoodRX. Many like like that system better. Dexcom prices can be found here:
https://www.diabeticwarehouse.org...e?q=dexcom

What is a CGM? They are small device that sticks on your skin for 10-14 days, with a small probe that sits under skin and continuously measures your glucose levels.

Bonus: United Healthcare is giving extra incentives depending on your plan. One is a $750 yearly deductible credit for wearing the meter for 20 days a year. The other is $0 covered co-pays, drugs, etc. You have to ask them which incentives you qualify for. From what I understand they do not stack.

Update 1: From what I understand the "Choice Plus" plans qualify for $750 deductible and the $0 Co-Pays are for Bind plan members. See attached.

Update 2:
Enrollment form:

If you have problems with the form, or it doesn't find your plan you can contact them here as shown in attachment:

Quote from CrazieBird :
FYI the typical requirement are:

The Dexcom G6 Glucose Monitoring System (K0553, K0554) will be considered for reimbursement for adult members when the below criteria are met:
• The member has established diagnosis of Diabetes Type I or Type II
• Medical necessity criteria are met (American Diabetic Association -2018; Centers for Medicare & Medicaid Services 2018 or NCD/LCD; Interqual
• The member is using a blood glucose monitor (BGM) and performing frequent (four or more times a day) testing; and,
• The member is treated with insulin multiple (three or more) daily injections or continuous subcutaneous insulin infusion (CSII) pump; and,
• The member's insulin treatment regimen requires frequent adjustments based on SBGM (self blood glucose monitoring) or CGM testing results; and,
• The member has frequent (at least 2 episodes within 30-day period) unexplained episodes of severe hypoglycemia (blood glucose less than 50 mg/dl) despite member's compliance with diet and insulin regimen adjustments/ OR hypoglycemia unawareness, requiring another person's resuscitative interventions (e.g., glucagon administration).
• Within six (6) months prior to ordering the CGM, the treating practitioner has an in person visit with the patient to evaluate their diabetes control and determined that criteria above are met.
That is what my friend ran into when trying to get UHC (United Healthcare) to cover his. They refused on the basis he didn't have insulin prescribed even though his doctor sent them documentation showing he needed it. He used the FreeStyle Libre 2 for about a year. Was able to get them through GoodRX for around $35 a sensor out of pocket (they last 2 weeks). He liked them. He then found out about Level 2 who is working with United, called them and they signed him up over the phone. They asked for address, phone, birthday and UHC member number and if he had been diagnosed with type II. Week later he had them. After seeing what he had to go through, I felt like I should post it as its such a huge savings and could potentially help a lot of people. I don't have time to make a lot of posts, so did my best. For what its worth, he likes the FreeStyle Libre 2 better, its about the size of a quarter. And Europe has the FreeStyle Libre 3 which is the size of penny. You can see the difference here:

it's a glitch in the matrix, everyone probably overwhelmed the system.

I haven't gotten diagnosed officially yet, but am pre and am trying to prevent the eventual diagnosis.

Anyone have UHC but don't need it, PM me.

Don't give up. I prevented by going on low-carb, low sugar and using MyFitnessPal/Fitbit to log in. I'll be 50 this year, am just about the only person I know without Type II yet am larger than them all (I lost 90 lbs in a year following this plan and doing exercise).

this reminded me to switch my parents from blue shield to something else. WOrth looking at united just for this since they both have diabetes

Don't switch to uhc just for this. In my experience uhc is one of the worst insurance carriers in terms of patient access and customer service.

But I'm sure you'll do your research. Maybe switching will end up being worth it for your parents' care if they have no other problems apart from type 2

You can get the FreeStyle Libre 2 for around $35 a sensor and they last 2 weeks out of pocket with discount cards like goodrx. A great idea for anyone with pre diabetes. Wearing one for for 1-3 months will help them understand what their diet is doing to their sugar levels. You do need a prescription, but most will give for pre diabetes. My friend liked the freestyle 2 better than the dexcom because it was smaller. But Level2 is covering his dexcom for free now.

I use the dexcom G6 and I have to tell you for the past 6 months it has been very very bad with the readings, they are meant to last 10 days, however it lasts anywhere from 5 to 6 then the readings will go all over the place and lose connection also not even coming close to what my meter with the finger prick says so be cautious when using this before using insulin and double check with your meter to make sure your blood sugar numbers are correct.

I wasn't in their list of "may be eligible" folks but for what they're offering, this is definitely worth checking out. CGMs are very expensive out of pocket, the Dexcom more so, they give you a Fitbit (not sure which model), and you have access to coaching so lots of benefits there.

I tried calling number twice and left voice mail but I am not getting a call back to enroll while form is throwing error. Anyone facing same thing? What has worked for you for this? Edit: I got the call back, rep was very helpful. She said she is unable to find me in the system so I might not be eligible for the program. I am part of choice plus plan with diabetes type 2 detected 2 months back via A1c 6.5 though while fasting glucose was normal. Doctor said let's do it again in 3 months instead of prescribing meds.
