I also suggest setting it up before Christmas day. Everyone else is going to be getting new hardware too. So things might be slow to update or just fail. Leaving your Switch possibly unplayable until you make an account or update.. Also you want to know it even turns on

You can create a Nintendo account at nintendo.com without a device. But in order to purchase, it looks like you need the "Nintendo Network ID" for that device to be associated with your account.

(I have a Switch but I don't have a 3DS. So, I tried buying a $1.00 3DS game to see if it would let me, and that's the error message I got.)

You may want to reach out to Nintendo Support to see if they can help you — obviously, it'd be nice to have all that squared away without having to completely unbox everything.

Lots of complaints about not being able to save the game. Is that still an issue?

My kids get frustrated trying to play this by themselves and figuring out which hero has to do what to move the story forward. Other than having to sometimes help them figure it out, they like the game

I have a switch in the closet ready to get gifted to my son for christmas. It's not set up yet, so neither is the online account. Can I purchase stuff like this, digital software ahead of time? Or do I need an account and switch already set up.

You need to set up a Nintendo account and buy it in the eshop

Unless something has changed, you need to have a Switch associated with your Nintendo account which does require opening and turning it on at least once. After you do that, you can buy from either the device or from the eShop web site and download the games later. I agree with the other poster that you should do this early to beat the Christmas Day rush (all the services get slammed that day and tend to be slow or even offline as it is basically a DDOS) as well as take advantage of BF sales, etc.
