Help with Macys rebate

Hello,

I see that Macy's has a free Tempur-Pedic pillow by way of using a mail-in rebate.

How do I get the rebate form?

I don't see it on slickdeals or on the Tempur-Pedic website [tempurpedic.com].

Can someone help me?

Click on the mattress you want.
Below the price there is text that says "Free Tempur-Pedic Pillows $300 Value" Click it.
Click "FREE TEMPUR-PEDIC PILLOWS $300 VALUE" to expand it.
Click "GET REBATE"
Expand "Current Rebate Forms"
Scroll to the bottom and you'll see "Tempur-Pedic Pillow Rebate"
