Costco kept cancelling my past orders of this gift card and others and it took me forever to figure why: they do not accept VOIP numbers (e.g. Google voice numbers).

Putting my real phone number allowed all my gift card orders to go through.

Hope this helps someone.

Same thing happened to me. I read in the comments that you can call Costco for them to fix it manually. Something about using a email or address not associated to the Costco member. Idk but it's SO frustrating.
