Have someone else signup (spouse, significant other) and put them as primary and yourself as secondary (domestic) and it will work. As long as the primary hasn't had a membership recently.

How do I get the 45$ cash card.
Yes. Was stuck on processing for about 5 hours before receiving confirmation email from Groupon followed by the email from Costco shortly after.
