I am not saying buying at Amazon will cause the issue, but it's been happening quite a bit when bought in local stores.
Just FYI.
If you've clicked on the main promotion link in the OP, your promo code will automatically be applied to your account. So re-applying it at checkout won't work. You will still qualify for the $10 credit if you purchase $100+ w/ the added blurb during checkout

Once you click the link and it says applied to account, you don't need to type in the code at checkout.
Buy it and after the card is delivered, you get an email saying the credit is in your account but won't appear in your gift balance. Just have to spend it before Nov 30.

I got both emails at the same time, one for the Google giftcard code and the notification of the $10 Amazon credit. Took like 5m from when I ordered to when I got the above emails.
