GFCI will "sense" the difference in the amount of electricity flowing into the circuit to that flowing out, even in amounts of current as small as 4 or 5 milliamps. This is is a measurement between hot and neutral. No ground required to function properly. You can even replace an ungrounded plug with no access to ground wire with a gfci to get the proper ground fault coverage.
The neutral and ground are bonded together at the electrical panel.
Most gfci testers are applying the fault between hot and neutral. An electrical inspector might have a portable gfci tester which applies the fault between hot and ground.
Whether the paper clip will explode depends on its size and the duration of the fault.
