The quality of life improvements are great such as showtime attacks, ammunition replenishes every battle, and much more than I can list.
They've also reworked mementos, added two new confidants which also really shape up the story.
New third semester adds 20-25 hours of extra gameplay with the new palace being my personal favorite. If you wish to play it, you will have to play through the entire first two semesters first (no way to start the third semester out of the box or transfer old saves)

I first beat the regular version of this game and did not want it to end, even after I had already put 100 hours into it. Then when they came out with Royal, I paid full price again (because it's worth it), and played through the whole thing again, since they added another 20 hours and a new character who has her own fleshed out story.
This game is just incredible.
P5R to me is like reliviing my high school days in a much much more colorful way.
I don't think that is the case for SMTV though I have not yet played it.
The combat is fun but not very complex.
The focus of P5R is on forging relationships as well as obtaining more powerful monsters.(the latter is similar to SMT I think.) P5R started really slow but become much more initeresting after the first maybe 1-2 hours. Also there are many romance options in P5R if that matters to you.
