Our system shows that your submission is registered with information not compatible with our site. Our corporate office has requested further information to determine if this submission can be released for activation.
Please provide us with verification of residency for "Name" at "Address" in the form of a business or government-generated document such as a utility bill or voter's registration card. Please note, we do not accept documents such as receipt or shipping invoices and statements. Do not send verification that includes personal information such as a driver's license ID number, Social Security, or date of birth.
Please email the appropriate document no later than 5 business days from receiving this email or the case will be closed.
As long as we can see verification for the information above then we would be happy to look into this further.

I would try to avoid this offer as they may just reject your submission even if the purchase is legitimate.
For example, I made a purchase at Sam's Club and the UPCs are on the list of allowed items. I have had it rejected for "not showing a qualified purchase" and "unable to verify the image as a valid receipt." I have tried to submit as an online purchase and as an in store purchase since it is an in store purchase through their self check out in the app. The rejections are extremely vague so it started to seem like a scam - I assumed they just reject everything but some people here seem to have had success I guess.
