I've never played this or know anything about it, but I qualify as a sometimes crude humor person.
Do I need a starter kit or something to work with this? Can someone make suggestions?

It's a party game, so the more people you have, the better the experience. Also, it's best if they like vulgar/potty humor too, because nothing brings down a game like having a prude or two try to play.
Edit: Also, you do not need the base game, you can just play by using different expansions, but the base is likely the biggest bang for your buck.

Also, the expansion packs are VERY American pop culture based. So if you're playing with someone who didn't grow up in the US, you might hear a lot of "I don't even know what this card is" out of people, like my European partner.
