Have two. Both had problems within a year with light use. RMAed one and got the same one back.

Yes, I purchased one in excellent condition. It was practically brand new with no issues. My partner purchased one also excellent condition but the bumper wasn't working properly. He opened it and fixed it using YouTube videos. Works great now.

You can purchase a 3 year extended warranty for $20, for up to 45 days after purchase from Xbox if you register the controller. If you have any problems early on you have 14 days to return it to Best Buy. So I think it's worth it.
