I do use inserts like that in my camera bag, however mine are discontinued Timbuk2 inserts into a Peak Design Everyday Carry backpack. Same concept but with smaller inserts I can have 2: one for my body, leash and batteries, and one insert for my 3 lenses.
TL/DR: I don't have this bag but the system is sound from a good brand.
I just took this bag to Iceland and Greenland for vacation. The insert is held in by friction, but it's quite snug. Downside is you can't pull it out easily, upside is it won't fall out easily. The tripod straps and pocket worked well, too. There were no manufacturing defects, everything looked finished well. At this price, would recommend.
I didn't use the strap for the insert, used PGY Tech quick connects instead so I could just have one camera strap to use on the camera or the bag. With the strap attached, the insert was too hard to get in and out.
Since it doesn't have a waist strap, could it cause shoulder pain when fully loaded?

Yeah but this one has a photo of a small camera bag sitting outside... I don't see that on Amazon
