I fly American often. IMO they don't give people a hard time often. For 2 inches I think you'll be fine as long as the thing isn't loaded and you can shove it under seat.

I usually carry extra clothes in my personal item backpack in case I need to remove some to "wear" and squish my personal item back to regulation.

This is apparently the XL version. The large version is $29 on the page and has better measurements. 18x12x7. With expansion it's 32-35L and a more realistic and consistent personal item size. It's what people should be buying if they want this particular backpack and don't want to bring a carry on.
