I asked support for help and here is their response (copied and pasted):
Hello,
We are sorry to see that you are having trouble with our code. Please follow these steps so that we can identify the issue and help you:
1 - Contact the Xbox Support team and ask them when the code was redeemed (the date and time). Don't forget to clearly write your code in the message.
2 - Once you get this information, take a screenshot of the entire conversation without cropping or editing anything.
3 - Finally, send the screenshot to us in here directly.
Once we receive this information, we will be able to find the exact issue and replace your code or refund you, whichever you prefer.

We thank you for your patience and apologise for any inconveniences we may have caused.

Now I'm out the money for the code and my only recourse is to do THEIR job and figure out whose code they sold me and when that person redeemed it. HARD PASS on anything from this crap company.
EDIT - I've used other sites like CDKeys and Eneba successfully several times. This was my first and last experience with "Instant Gaming".

Judging by other experiences online with this company, I'm not alone. Instant Gaming customer service stinks.
