ELIGIBILITY: The CALIFORNIA PIZZA KITCHEN'S 2025 THANK YOU CARD PROMOTION ("Promotion") is open only to individuals who are legal residents and physically located in one (1) of the following U.S. States ONLY: AZ, CA, CO, CT, FL, GA, HI, IL, KY, LA, MA, MD, MI, MO, NC, NJ, NM, NV, NY, OH, OR, PA, TN, TX, VA, or WI, who are at least eighteen (18) years old as of date of participation, and who either live or work within a 25-mile radius of a participating CPK restaurant. Promotion is valid at all participating CPK restaurants within the US excluding CPK airport locations, CPK's Mobile Kitchen, or non-participating CPK Franchise locations
TIMING: The Promotion begins on March 1, 2025, and ends on June 8, 2025, at the close of business of the participating CPK location
AMOE: To request a Card without making a purchase or visiting a CPK restaurant, hand-print your full name, full mailing address, email address, phone number, and birth date on a plain 3x5 inch paper or card. You must include a self-addressed, stamped envelope ("SASE") with proper postage which will be used to send you a Card via return mail.
