I got this a few weeks back. I am not sure if it just my sample or Lodge quality is not what it used to be. The one that I receive has file mark (?) on the outside of the lip. Since these are casted, I suspect the mold might be worn. Also, the seasoning from the factory is uneven. There is a thick layer on one side of the handle. I have a 12" pan that I got about 10 yrs ago and didn't have these problems.

The file mark is pretty common. I would say its on ~half of the lodge pans/bakeware/accessories I own. The uneven seasoning is not something I have come across, but its probably because of the way it was hanging when it was sprayed and baked on.

I have the larger one that I use as a griddle/flat top on my kettle grill. Fits great and easier clean up than a designated flat top cooker.

Depends on what you want out of it. For stovetop cooking you probably want the "standard" one with a handle and the assist handle. This model, with two assist handles, is better for oven or BBQ use, since it fits better and you can generally pick it up easier with better balance for long distance moves.

I really like this one because it can fit in my toaster oven, and I've made some great foccacia bread in the toaster oven that way. But I believe it's about the same size as the one in the main deal, so either would work.

If you have a toaster oven, that's a compelling reason to get one of these over the normal ones with the longer handles--those won't fit.

Yes. The loop handles are often called baker's skillets.

They're also awesome for using on the grill, for cooking side dishes and the like. The long handle is often an obstruction you don't want to deal with. This is a great size for a 22" weber.
