After much research and forums Reddit directed me to an Australian vet pharmacy where you do not need RX. Hopefully not against forum to suggest. Just bought a year supply of Simparica Trio (11-22lbs) for $108 + $10 DHL shipping.

What a relief, because shame on these companies charging so much for preventative RX in the US.
I was using Costco as well because they seemed to be the cheapest, then decided to do a bit of research and I found this site on a reddit thread as well. They saved about $300 for my dog and cats meds for the year.
Goes to show the difference between a government that regulates pharmaceutical prices vs a free for all.

Most vets are not nutritionists. They are brainwashed in college by companies giving them deeply-discounted food:

We have our dogs on ProPlan, Science Diet, etc as we shifted to WSAVA approved foods after all 3 of our Goldens were getting diagnosed with DCM. All 3 recovered, and now we only serve WSAVA or other large brand foods.
2 are super healthy. And our newer 2 are as well. Ages 11, 9, 5, 3.

"While Zoetis is a global company with markets in multiple countries, we are not able to support products that have been produced for foreign markets and then imported by third parties for use in the United States, as product integrity and handling cannot be guaranteed. Use of these products would be at your discretion, and they would not be covered under the US Simparica™ Satisfaction Guarantee. For purchases fulfilled with foreign products, we can track a lot/batch number to determine if the lot/batch is valid; however, this would be the extent of the information we can provide. If you would like to send me the lot number for the product you purchased, I can check it with our manufacturing department to verify if it is a Zoetis product and for which country it is manufactured, but again that would be the only information I could provide regarding a foreign product.
With regard to product distribution, Zoetis sells directly to authorized licensed channels only, including veterinarians with a valid Zoetis account. Your veterinarian continues to remain the best place to purchase our products. Because Simparica® (sarolaner chewables) is regulated by the FDA, a valid prescription is required, authorized by a veterinarian with a valid client-patient relationship. This ensures that the product, the product dose, and directions for use are accurate and correct for your pet and the desired indication. While we do not have a list of online vendors that can be provided, Zoetis will support the product from an online purchase if it contains a product marketed for the United States, from a reputable US-based website, that requires a valid veterinary prescription.
For purchase options, please consult with your regular veterinarian for recommendations. Should you have additional questions regarding Zoetis products, please contact Zoetis Veterinary Medical Information and Product Support by calling 888-963-8471, option 2."
They can check the LOT# but will not honor the guarantee program in US. Not sure how much that coverage consisted of.
