I got the 30qt in the subtrunk of my Y, it lifts the trunk mat just a tad but hardly noticeable. The 23qt fits without lifting the trunk mat but I found it was too small. New Tesla firmware update lets you keep accessories on without having to use camp mode so now I leave it on 24/7. Works great, I use it often.
Absolutely FANTASTIC little fridge. Not only did it perform flawlessly during the two months of summer travel, but I gave it to my parents after the summer and they've used it since as a drink fridge in the entertainment room. I left it plugged in to a powerbank that has a 200w solar panel. That's enough to keep it running at 45f the whole day.
One thing -- the AC adapter SUCKS. It will eventually stop supplying enough voltage and the fridge will throw an "E3" (compressor) error code so matter how much you tinker with the settings. Thankfully I had an old desktop computer from which I pulled the power supply and made a proper 12v power supply.

You wouldn't want it in there even if it fit because you want a larger volume of air to dissipate the heat the cooler is moving.
Eh, the heat from a 45W heat pump is nothing. The trunk would have no trouble dissipating that, though the air around it will be noticeably warm. There will be a slight drop in efficiency, but I doubt it's a deal breaker. There's also this review from the product page:
I think it just depends on your specific needs. $50 extra for some extra height could come in very handy if you live somewhere that gets super hot and you need to drive with cold food or drinks/leftovers for longer than 20-30 minutes.
I drive a lot for my job and am contemplating the 30qt "pro" version because it seems like it would be big enough for some drinks and food without having to constantly worry about ice melting issues or being forced to rely on gas station food and drinks. Also the second compartment on the "pro" could be for napkins and utensils etc. Another reason im considering the 30qt pro version though, is the extra height seemingly lets most types of bottles stay upright and should reduce the risk of stuff spilling if i want a bit more heat/time flexibility for grocery shopping. Another reason specific to my needs is that it seems like it should hold takeout/to go boxes if i want to stop for food while im out for work. For myself, i think the dimensions of the 30qt sizes would def be more functional and therefore valuable without adding too much to the bulkiness of the item.

It's very quiet, and seems pretty power efficient. I get about 2 days of runtime off a EcoFlow River 2 Pro (756 Wh) when my vehicle isn't running.
One thing to note: the 23 qt is a little too short to easily take 1 gallon milk jugs, 2 liter bottles, frozen pizzas, etc. For that reason I would have preferred the 30qt which might be the sweet spot for capacity vs. space, but couldn't get one shipped to Hawaii.
There was a known issue with the LCD display dying on these, but it seemed Bougerv was very good about sending replacements and it's a very easy fix. Also, as expected the internal temp control isn't very accurate which I solved by velcroing a Bluetooth temperature logger to the inside of the lid. Between uses I use the logger to prop the lid open to avoid moisture buildup/mold.
There is a newer version that has an internal light, tie down anchors, and some other upgrades but I haven't had a chance to try it out. There are also versions which will accept a proprietary 256 Wh battery, but I think a jackery/EcoFlow is a cheaper and more versatile option.
All said, I'm happy with mine and would easily recommend it, especially at this price.
