Difficulty rewards only unlock on the specific difficulty and the highest difficulty does not unlock the lower difficulty rewards so if you want the cosmetics you'll have 3 playthroughs unless it gets patched.
You'll likely have 3 favorite weapons and a favorite shield rune.
I look forward to story dlc.
First playthrough unlocking all upgrades, getting all secrets and challenges was on nightmare was the longest.
Second playthrough Ultra Nightmare (perma death) was a lot less stressful with the sliders enabled to deal 1000% damage, make enemies attack less and deal less damage.
Third on Pandemonium was the real chore of doing it a 3rd time.
Game does glitch/crash so I had to do an entire level again 2x which is an inconvenience

I get why it may have been done - Increase replayability. But games beforehand let you score the lower difficulty rewards if you did play a higher one, so this change is rather odd to do after so many years.
I also want to mention that the game on physical is NOT a physical copy. It's a digital license on a disk that unlocks the full download of the game. A prior Call of Duty from a couple years ago did the same thing with their console disks. So if you want to own this game for the long haul, then be considerate that your disk won't let you install a base or 1.0 copy of the game in full without internet.

The modern Doom releases are Doom (2016), Doom Eternal, and Doom the Dark Ages. They're all really well made and worth playing, in my opinion.
The game makes you use a shield to have the player become passive. Doom slayer (once Doom Guy) bends the knee to a female leader so powerful that it makes no sense as to why she doesn't solve the problem other than influencing the player to do unnatural bidding. In fact, there is even a reinforcement through voice commands by Debra Wilson (woke queen) in order to get you used to it. Last but not least, Mick Gordon is not on there to make the soundtrack. There are players who give choppy reviews and those who give thorough reviews. The most common consensus from the thorough reviewers is that this game is Doom without the soul of Doom.
That's why the game is on sale.
